Kelsey has spearheaded initiatives that prioritize both operational efficiency and team engagement. She successfully implemented the "Leadership Team Boost" weekly huddles, ensuring that every department stays aligned with our mission and strategic goals. While navigating the corporate "Project ImPACT" cost-cutting measures, she maintained a sharp focus on labor budgets and overtime management. Despite these fiscal pressures, she achieved a remarkable 9.0% contribution margin in recent months, demonstrating that excellence in care and financial health can go hand-in-hand.

Kelsey’s vision is best exemplified by her hands-on approach to resident care and interaction, never asking her staff to do things she herself would not do or help with. Furthermore, her leadership in adopting the "4Ms" of Age-Friendly Care — focusing on What Matters, Medication, Mentation, and Mobility — has ensured that our evidence-based practices are deeply integrated into daily resident life. She additionally revamped our QAPI program to ensure continuous improvement included all disciplines and departments. As a result, St. Leonard has exceeded quality goals and improved its CMS quality rating from 3 to 5 stars in one year.

Under Kelsey’s guidance, St. Leonard has attained a prestigious five-star rating from CMS. This achievement is a direct result of her "aggressive intervention" strategies regarding quality measures, such as fall prevention and clinical follow-up. Kelsey is a natural leader who lives the mission of CHI Living Communities daily, fostering a culture where every resident’s goals are honored and every staff member’s contribution is valued.

Use of OnShift in our organization helped Kelsey monitor staffing and overtime use on a daily and weekly basis. The reports generated were essential to also preemptively identifying and resolving potential overtime use by rearranging staffing patterns.

Kristen Andrews Birdsong

Kensington Health & Rehabilitation Center

Kristen Birdsong is the embodiment of transformational leadership in long-term care. Through vision, accountability, and an unwavering commitment to excellence, she has led an extraordinary turnaround that has positively impacted residents, families, employees, and the community we serve. Her ability to inspire others while delivering measurable results makes her an exceptional candidate.

Joining our organization in 2017, she inherited a facility facing significant challenges. At that time, the facility held a 1-Star quality rating and cared for only 70 of its 120 licensed beds. Rather than accepting the status quo, Kristen established a culture of accountability, teamwork, and excellence. Today, under her leadership, the facility has earned a 5-Star quality rating and consistently maintains approximately 95% occupancy. This transformation reflects not only operational success but also the trust that residents, families, referral partners, and the community have placed in the care provided by her team.

Kristen understands that outstanding resident care begins with investing in people. She has successfully transformed the workplace culture by recruiting and retaining high-caliber employees, creating more balanced workloads, and fostering an environment where staff members feel valued, supported, and empowered to succeed. She has established clear expectations while cultivating a culture rooted in professionalism, accountability, and mutual respect. The result is a stronger, more engaged team that takes pride in the care they provide and is committed to a shared standard of excellence.

Her leadership is perhaps most evident during times of adversity. Upon becoming Administrator, Kristen led a complete redesign of the facility’s staffing model to ensure a smooth transition and provide unwavering support to her team. During Alabama’s historic snowstorm in 2025, she stayed overnight in the facility alongside her staff, ensuring residents received uninterrupted, high-quality care despite hazardous conditions. These moments exemplify her servant leadership and willingness to place the needs of others above her own.

Kristen does not simply manage a nursing home – she develops leaders, builds strong teams, and creates a culture where excellence is expected because every resident deserves nothing less. Her integrity, resilience, and commitment to continuous improvement have transformed not only the facility’s outcomes but also the lives of the people who live and work there. Kristen Birdsong has demonstrated what is possible when leadership is guided by purpose, compassion, and accountability. Her extraordinary accomplishments and lasting impact make her exceptionally deserving of recognition.

Many of our initiatives included the improved utilization of OnShift as a primary resource. Once the system was utilized properly and proper staff education provided, our processes became streamlined like never before.

Kristen's mottos are to "Always Do The Right Thing" and "We Don't Grow Employees, We Build Leaders". This award nomination is due to her commitment to seeing these through every day.

Tessa Parks

Renton Health & Rehabilitation

Tessa deserves to be recognized for consistently finding innovative ways to improve staffing stability, workforce satisfaction, and operational efficiency. Through strategic scheduling practices, proactive workforce management, and a strong focus on employee engagement, she has created a workplace culture that supports both staff and residents.

One of Tessa’s most significant achievements has been reducing overtime from an average of 5-6% to below 2%, resulting in more efficient labor utilization while maintaining quality resident care. She has also helped build and sustain a stable leadership and nursing workforce. While retention metrics can be difficult to quantify directly, our facility maintains a 4-Star CMS Staffing Quality Measure rating, supported by exceptionally low administrative turnover. Additionally, our RN Staffing Hours and Weekend Staffing Hours both hold 5-Star ratings, demonstrating commitment to maintaining consistent staffing levels and ensuring quality care coverage at all times. Tessa's ability to balance workforce needs, quality outcomes, and labor management has had a measurable positive impact on our organization and makes her highly deserving of this award.

OnShift helped support these initiatives by providing scheduling, workforce management, and staffing visibility tools that enabled more efficient decision-making. The platform helped identify staffing gaps, improve schedule coverage, reduce unnecessary overtime, and streamline communication with employees. These capabilities supported Tessa’s efforts to improve workforce stability and operational efficiency.

In addition to operational improvements, Tessa demonstrates a genuine commitment to employees and residents. She consistently leads by example, remains approachable and supportive, and focuses on creating a positive workplace culture. Her ability to balance employee needs with organizational goals has made a meaningful impact on team morale, retention, and overall workforce performance.

Dekenya Alexander

Quality Center for Rehabilitation & Healing

Dekenya has been an amazing asset to our team in the short time she has assumed the role of Staffing Coordinator. In just one year, she has brought CNA agency usage down to less than 10%. Dekenya has helped increase CNA retention up to 92% over the last 6 months. She manages overtime exceptionally well and has maintained it at 10% or less.

Dekenya communicates very well with our staff and has an open door for any questions or concerns. She has learned the ins and outs of OnShift and is able to train all new employees on the system as well as troubleshoot any needs.

With the ease of reviewing reports, managing time-off requests, and communicating with staff, Onshift has helped make it possible to enjoy scheduling for all employees.

Monique Holland

Good Samaritan Society - Comforcare

It is hard to explain what Monique does for our facility because so much of it happens when no one is watching. Monique doesn't simply build a schedule in OnShift and walk away from it. She lives it. She is constantly watching staffing needs, managing open shifts, communicating opportunities to staff, adjusting for call-ins and changes, and using OnShift to keep our schedule organized and our building covered.

When something changes (and in long-term care something always changes) Monique is already working on the solution. It happens at 10:00 at night. It happens early in the morning, on weekends, and on holidays. When the rest of us see a hole in the schedule and wonder how we are possibly going to fill it, Monique somehow finds a way.

What makes Monique exceptional is that she understands the people behind the OnShift schedule. She knows our employees, their availability, their strengths, and who may be willing to help when we need it most. She uses that knowledge alongside OnShift to make thoughtful staffing decisions rather than simply filling empty boxes. Monique embodies our values of Service, Courage, Humility, Stewardship, and Family in the way she manages our workforce every day.

Her results are measurable. In July, our facility finished $3,329 under budget in overtime salaries and $9,833 under budget in incentive salaries, for a combined $13,162 under budget. Those numbers represent effective workforce management, but they don't show the countless open shifts filled, schedule adjustments made, staffing problems prevented, and hours Monique has given to making it happen. 


OnShift gives us the tool to manage our schedule. Monique is the person who turns that tool into a workforce that works. There are people whose work helps an organization run, and then there are people whose absence would change the heartbeat of the entire place.  Monique is the latter.

Monique's impact cannot be fully captured by a schedule, a budget, or a staffing report. Behind every successfully filled shift is a resident who had someone there to care for them and an employee who knew someone was working just as hard to support them. Monique gives an extraordinary amount of herself to this role. She answers when no one expects her to answer, solves problems most people never even knew existed, and rarely asks for recognition for any of it. As the Director of Nursing, I can quite honestly say that I could not do my job without her. Knowing Monique is watching our staffing, anticipating problems, and doing everything she can to keep our building covered allows me to focus on our residents, our nurses, and the clinical needs of our facility. That kind of partnership is invaluable.

If you want to understand Monique's impact, look beyond the schedule. Look at the employees who trust her, the leaders who depend on her, and the residents who ultimately benefit from the work she does behind the scenes. We would feel her absence immediately. We are incredibly fortunate to have her.

LaToiya Remy

Legacy Pointe at UCF

LaToiya is amazing and in a few short months was able to help us virtually eliminate expensive agency staffing, decreasing it by 80% thanks to her efforts. She works the floors, gets to know the employees and leads with kindness and understanding. She is an extremely organized and valued team member and we love her!

Switching to OnShift has helped us fill shifts internally rather than using agency and has added a layer of fairness for all employees to eliminate any claims of favoritism.

Dr Martha A Klima

Plymouth Place

Martha was a key decision-maker in bringing OnShift Schedule to Plymouth Place and played an important role in its implementation. Moving to OnShift helped us modernize processes that had previously been much more manual and gave our employees and leaders better visibility into schedules, timekeeping, PTO, attendance, and staffing. For Martha, implementing OnShift was not simply about introducing new technology – it was about giving our teams better tools, improving efficiency, and making everyday workforce processes easier and more accessible for our employees. OnShift has become an important part of how we manage and support our workforce today.

Martha has had a positive impact on the people who work with her every day. I have the privilege of being part of her People Services team, and she has created an environment where we are trusted to bring ideas forward, challenge old processes, continue learning, and find better ways to support our employees. Martha is the kind of leader who will celebrate a major organizational accomplishment but will give the same attention to helping one employee who walks into our office needing support. That example has shaped the way our entire team approaches HR. Her impact is multiplied through the people she leads. She has built a strong team that shares her belief that employees should feel heard, supported, and valued, and she pushes us to keep raising that standard.

The numbers show what Martha has accomplished. What they cannot fully show is the influence she has had on our culture and our employees who have become better because of her leadership. 

Shelly Lee

Nicholasville Nursing & Rehabilitation

Our HR director Shelly Lee truly goes above and beyond. When I started in March 2026 she stated to me as the DON "you take care of the residents and I will take care of our team members." I have never seen anything like it! She provides a hot meal to our staff every Wednesday such as potato bar, burger bar, pizza and chef salad. We have several nursing assistants that come to work hungry and live on gas station drinks and food.

The hospitality that is provided by Shelly truly goes above and beyond. She will stop what she is doing to process a new hire even though she may have piles on her desk and multiple reports due. She makes that team member feel like they are the only person in the world. She always keeps OnShift updated and works feverishly to ensure our reports from OnShift are accurate. She is always willing to stop and teach employees how to use the app, and teach managers how to use the multiple reports and data available to us. Due to her efforts, OnShift has played a major role in ensuring we are adequately staffed as well as communication is vital to ensure our employees know all openings available.

By Shelly keeping OnShift up to date, we have all shifts covered for our residents and never work short staffed. We continue to review the up to date data provided by Shelly and OnShift to ensure we have a great retention at reduced cost. With the reports from OnShift we can choose the most cost efficient employee to fill the shift.

When you think of an HR director who is 100-percent focused on the team member, ensuring all data is up to date to promote the most efficient process to manage staffing in our 73-bed skilled nursing facility, you think of Shelly Lee. She is the best!

Mark Loy

Buckeye Care & Rehabilitation

Mark understands that Human Resources in long-term care is ultimately about people. Policies, recruitment, retention, attendance, benefits, onboarding, and employee relations are all important, but Mark recognizes that behind every one of those processes is an individual employee who deserves to be heard, respected, and supported.

One of Mark’s greatest contributions to our community is his accessibility. Employees know they can approach him with questions, concerns, or problems, and he makes an effort to listen and help identify a solution. In an environment that operates 24 hours a day, seven days a week, having an HR leader who remains connected to the workforce is incredibly important.

Mark also works closely with department leaders to address one of the greatest challenges facing long-term care: building and maintaining a reliable workforce. He supports recruitment, onboarding, retention, employee relations, attendance management, and efforts to reduce dependence on agency staffing. Rather than viewing these as separate HR functions, he understands how interconnected they are. Recruiting an employee is only the beginning; creating an environment where that employee feels supported and wants to remain with the organization is equally important.

He has also helped us strengthen expectations around reliability and accountability while maintaining an employee-centered approach. Those two concepts are sometimes treated as opposites, but Mark understands that a healthy workplace needs both. Employees deserve consistency, communication, fairness, and support, while residents and coworkers also deserve a team they can depend on.

Mark collaborates closely with leadership when workforce challenges arise and helps us consider not only what is operationally necessary, but how decisions will affect our employees. His perspective has helped make our organization a better place to work and has strengthened the partnership between Human Resources and operational leadership. Mark's impact can be seen every day in recruitment efforts, employee support, workforce problem-solving, and the culture we are continuing to build.

OnShift supports many of the workforce initiatives that Mark and our leadership team are working to strengthen. OnShift provides a centralized platform for scheduling and workforce communication, helping employees see staffing opportunities and allowing leadership to communicate open shifts more efficiently. This is particularly valuable in a 24/7 healthcare environment where staffing needs can change quickly.

The information available through OnShift also supports conversations about attendance, scheduling, staffing patterns, and workforce needs. Having greater visibility allows HR and operational leadership to move beyond individual staffing events and identify trends that may require recruitment, retention, scheduling, or employee-relations interventions.

For Mark, OnShift complements the human side of HR. Technology can make workforce processes more efficient and transparent, but Mark provides the personal communication, accessibility, and support that technology cannot replace. Together, those elements help us create a more responsive workforce-management system.

HR professionals in long-term care occupy a uniquely challenging position. They advocate for employees while supporting leaders, maintaining organizational standards, recruiting in an extremely competitive labor market, addressing difficult employee situations, and helping ensure that enough qualified people are available to care for residents every day. Mark handles those responsibilities while remembering that employees are people first. He is willing to have difficult conversations when necessary, but he is equally willing to listen. He understands that supporting employees does not mean eliminating accountability, and accountability does not mean abandoning empathy. That balance has made him a valuable resource for both employees and leaders.

Perhaps most importantly, Mark understands that culture is not created through a policy or an employee appreciation event. It is created through the everyday interactions employees have with their organization: whether someone listens when they have a concern, whether expectations are communicated clearly, whether people are treated fairly, and whether employees feel that their contribution matters. Mark helps create those experiences every day. His work supports the people who care for our residents, and the impact of that work ultimately reaches far beyond the HR office.

Kaylynn Crawford

Bellaire at Stone Port and The Legacy at North Augusta

Kaylynn Crawford is truly deserving of a Best in Care Award because of her dedication to our residents, her commitment to quality care, and her desire to continually grow as a healthcare professional.

At only 19 years old, Kaylynn has already demonstrated an impressive level of maturity, compassion, and responsibility in her role in assisted living. Kaylynn has been a certified nursing assistant since 2024 and became a registered medication aide in 2025. She has embraced both roles with a strong commitment to providing safe, respectful, and compassionate care to our residents. She consistently puts the needs of our residents first and understands that quality care is about much more than completing tasks — it is about making each resident feel valued, respected, and cared for.

Kaylynn is also investing in her future and in the future of healthcare by currently attending Blue Ridge Community College and taking nursing classes. Her willingness to further her education while working in assisted living demonstrates her passion for the profession and her commitment to becoming an even stronger caregiver.

Kaylynn makes the lives of those around her better simply by caring. She has a natural compassion for our residents and treats them with kindness, patience, dignity, and respect. She takes the time to get to know them as individuals and makes sure they feel valued and cared for.  One example that stands out to me personally is seeing Kaylynn come into the community on her day off to spend time with residents who were nearing the end of their lives. She was not scheduled to work and was not coming in for a work responsibility. She came because she wanted to be there for them. She would sit with them, visit with them, and provide companionship during a very difficult and meaningful time. To me, that speaks volumes about who Kaylynn is as a caregiver.

Since joining our community, Kaylynn has been a positive addition to our team. She contributes by being dependable, willing to help, and dedicated to the residents we serve. She brings a positive attitude to her work and demonstrates the kind of compassion and work ethic that makes a difference in the lives of our residents and in the workplace. Kaylynn represents what it means to be a dedicated caregiver. Her accomplishments at such a young age, combined with her continued education and genuine commitment to our residents, make her an outstanding example of the kind of team member who makes our community a better place.

At only 19 years old, Kaylynn has already developed an understanding that quality caregiving is about so much more than completing a checklist of duties. It is about being present, providing comfort, listening, showing compassion, and making sure our residents know they are not alone. She also makes a difference in the lives of her coworkers by being willing to help, maintaining a positive attitude, and being someone her team can count on. Kaylynn's kindness and genuine concern for others have a positive impact on our entire community. She doesn't just provide care, she creates meaningful connections, and that is something that cannot be taught in a classroom.

Tori Newman

Oak Grove Christian Retirement Village

Tori is a quiet and reserved person who doesn’t seek attention or recognition, but we constantly hear about her through the residents and families she cares for. We receive so many compliments about Tori and the care she provides. Residents adore her, families trust her, and we even have residents who specifically request Tori because of how safe, comfortable, and cared for she makes them feel.

One example that truly shows Tori’s heart happened when one of our residents was preparing to attend a funeral during an incredibly difficult time in their life. Tori came into work on her day off simply to help that resident get ready. She wasn’t looking for recognition or praise. She did it because she knew that resident needed someone, and she genuinely cared. That is who Tori is. 

You don’t hear Tori talking about all the things she does, you hear about them from the people she cares for. Her work, her heart, and her actions all line up. She quietly shows up every day and gives our residents the kind of compassionate, dignified care we would all want for someone we love. Tori is truly one of those caregivers whose work speaks for her, and I cannot think of someone more deserving of recognition for the difference she makes in our residents’ lives.

Trayvond Ray

Waltonwood University

Trayvond deserves to be recognized as a Best in Care Award winner because of the consistency, compassion, and pride he brings to caring for our residents. Over his nearly 10 months with our community, he has become someone residents, associates, and leadership can depend on.

His commitment to quality care can be seen in the small details. Trayvond makes sure residents are clean, bathed, well-groomed, and presentable while always treating them with dignity. One moment that stands out was when a resident's granddaughter was getting married. Trayvond took the extra time to help the resident get ready, including helping him with his suit and tie so he could look his best for such an important family occasion.

Trayvond has also made our resident care team stronger. He is a phenomenal trainer who takes new caregivers under his wing, especially those who are new to caregiving. He leads by example, helps them become comfortable and confident on the floor, and shows them the level of care we expect for our residents.

His impact is also felt by his teammates. During difficult shifts or staffing shortages, Trayvond is someone others can count on to step in and help. His willingness to support the team takes pressure off those around him and helps ensure our residents continue receiving the care they deserve. 

Trayvond has an exceptional level of commitment, not only to our community, but to Waltonwood as a whole. Just this week, I received a call from another Waltonwood community asking if we had a caregiver who would be willing to help them with staffing over the weekend. When I asked Trayvond, he agreed to help without hesitation.  That willingness to step up is nothing new for him. This past winter, when our own community was experiencing staffing challenges, Trayvond volunteered to stay over and work the floor alongside me to make sure our residents were taken care of. In nearly 10 months with our team, Trayvond has never called off for a scheduled shift. What makes that even more impressive is that he also works another caregiving job from 3 PM–11 PM. Despite balancing both jobs, he continues to show up, support his teammates, train new associates, and provide dependable care to our residents.

Trayvond's genuine care for our residents, willingness to support his teammates, and commitment to doing things the right way have made him an invaluable member of our community and an excellent example of what Best in Care represents.

Amanda Souhrada

Good Samaritan Society - Tyndall

Amanda has been employed with Good Samaritan Society Tyndall since December of 1998.  Her 28 years of dedication to our residents and facility have been outstanding.

Through these 28 years she continues to find ways to advance her knowledge and to advance the ways she can give of herself. She has held a position in almost every department and if she hasn't officially held a position in a department she is always finding ways to help that department whether it be doing laundry, mowing the lawn or curling hair. Her goal is quality of care to the residents and families we serve and if she can do something to improve that quality she will find a way to do that.

Amanda holds certifications as a certified medication aide, certified nurse’s aid, restorative aid and has added activity coordinator to her credentials these last couple years. She had been our main scheduler for many years and made sure staff were available for our residents. Working "short" was not an option for Amanda.  She would say "our residents don't deserve that." If there was an open shift, Amanda is there. There is no one else that has stepped up to a challenge with such grace more than Amanda. 

Amanda continues to find ways to improve herself in order to serve our residents and facility to the best of her ability. Her most recent addition to her credentials is being one of our SAFE coaches. She is finding ways to make sure our environment is safe not only for our residents but also for our staff.  She is active in our QAPI program as well.

As the activity coordinator, she is finding ways to bring the outside to our residents. Examples are having students from the school come at holiday time to sing for the residents, in the spring having students come and plant flowers with the residents, finding volunteers to play live music for the residents, finding ways to bring pets into the facility, and doing ice cream runs so our residents can enjoy the gifts that the community has to offer. The list could go on and on of the ways she gives of herself to improve the lives of not only our residents but also our staff. 

Amanda has given of herself for so many years in so many ways and never asks for anything in return. She is very humble in what she does and only wants the best for our residents. Her heart and soul goes into not only our facility but our community. Even with her full time job here she finds ways to be involved in our community, helping bridge the community involvement in our facility. Amanda deserves this award for the many years of dedication she has given to our residents and the humble loving care she delivers every day.

Zina Lee

Sunrise of Frederick

Zina deserves to be a Best in Care Award winner because she represents what dedicated caregiving should look like. She has devoted 19 years to Sunrise at Frederick, and throughout those years she has remained dependable, compassionate, professional, and committed to providing exceptional care.

Zina does much more than complete the responsibilities of her position. She pays attention to the individual needs of each resident and takes the time necessary to provide quality, person-centered care. She communicates with families, participates in family meetings when asked, helps ensure needed supplies are available, and shares her experience by helping train new team members. She is someone we can count on without hesitation.

Perhaps one of Zina's greatest contributions to our community is her consistency. She shows up. She doesn't complain. She doesn't involve herself in negativity or drama. Instead, she focuses on our residents and on doing her job well. For nearly two decades, Zina has quietly and consistently demonstrated the values we want every caregiver to possess. Her commitment has helped make Sunrise at Frederick a better place for residents, families, and team members alike.

What stands out most about Zina is the way she cares for our residents. She never makes them feel like they are simply another task that needs to be completed. She takes her time, treats each person with dignity and respect, and provides care as though each resident were a member of her own family. That compassion brings comfort not only to our residents, but also to their families, who can feel confident knowing their loved one is in caring hands.

Zina also makes life better for the people she works beside. She steps in wherever she is needed, is always willing to lend a helping hand to her coworkers, and simply comes to work ready to do what needs to be done. Her steady, positive presence makes a difference every single day.

In caregiving, there are often people whose contributions aren't defined by one extraordinary event, but by thousands of ordinary moments in which they choose to show up, be patient, be kind, and give someone the care they deserve. Zina is one of those people.  For 19 years, residents and families have been able to count on her. Coworkers have learned from her. Leaders have trusted her. She has cared for countless residents and treated them with the same compassion and respect she would want for someone in her own family. Zina does not seek attention or recognition for what she does. She simply comes to work, smiles, cares for her residents, supports her team, and does what needs to be done. That quiet, consistent dedication is exactly why I believe she deserves to be recognized.

Jean Marc Remy

Lincoln Meadows Senior Living

Jean Marc, aka Marc, is one of the kindest and most genuine caregivers we have here at Lincoln Meadows. His calm demeanor, patience and attentiveness to the residents is unmatched.
Marc is always on top of picking up meal orders for his assisted living residents, ensuring they receive their meals in a timely manner and warm. If I or any of my servers need assistance with an assisted living resident, he is always happy to help whether it is his resident he is responsible for or not. 

We have a highly non-verbal assisted living resident and it is very difficult for my young servers to take her order. Sometimes it takes as much as 10-to-15 minutes to get the order. Marc spends a lot of time helping this resident decide on what to eat. He uses our picture menu as well as non-verbal cues with her such as 1 finger for yes, 2 for no. Marc is a gem for his residents and those that work with him.

It is nice to have someone like Marc that is dedicated to our residents.
